Tests & Blank Answer Sheet for Accredited Home-Study Programs
 

If you have one of our training manuals, courses, videos, cd-rom or home study courses, you can complete and return the appropriate post-test for credits and a certificate. These are provided to registered users of these programs, or employees if it is the institution.  These are in Adobe .pdf format so you must have that on your computer first. If you don't click the icon below to download a free copy, then come back here for the tests.
You must pass with a 70% or higher score to gain the Credits and Certificate of Laser Training, but can retake the tests any number of times at no charge if needed.


Note that some States may require a passing grade of more than 70% in order to meet their licensing requirements. In those states that have such a requirement will will be adjusting our passing score to match the State's, for those applicants that list that State as their address. (Arizona requires 80% for the aesthetic program)

  • BLANK ANSWER SHEET FOR ALL TESTS - Use this bubble sheet to mark your answers for the tests. Ensure that your name, date and contact phone  number is placed at the top of each answer sheet.  Ensure that the test name and version number (if applicable - i.e. v060303) is written on the answer sheet. The version numbers start with the letter v and are located at the top of the test on the first page. This answer sheet has space for 155 answers, but most of our tests are not this long so you will have many blanks at the end.

    TESTS & REGISTRATION FORMS FOR CERTIFICATES
    Each of the tests below has a registration page with it. Fax that registration page, plus the completed answer sheet (above) to 305-946-0232.
    DO NOT fax the entire test to us - just the registration page and the completed answer sheet.
     

  • Fundamentals of Laser Safety - VHS Inservice Video - (Video still pending - not available)
         30 minute video, plus 20 minutes allotted for the post test.  1.0 Contact Hours and Certificate of Laser Safety Training. Certificates and Credits for 5 people are included with the purchase of the Video, additional Certificates and Credits are available for $35 each to registered users and their employees.

  • Lasers in Medicine & Surgery - Electronic Course Manual in .pdf format - (Also for InHouse Training)
    174+ page color illustrated manual divided into sections for testing. Complete all 5 post tests for 16.1 Contact Hours and Certificate of Laser Training. Certificates and Credits for 2 people are included with the purchase of the manual on CD-ROM, additional Certificates and Credits are available for $85 each to registered users and their employees. This CD-ROM is provided free of charge to our standard InHouse Laser Training clients, and other employees in the institution may read the manual and take these tests for ongoing inhouse certification of employees ($85 ea).

  • Aesthetic Laser Procedures Home Study Course - 41.2 contact hours, 140 questions taken from the 234 page color illustrated Aesthetic Laser Procedures Home Study Manual. Includes registration page & answer sheet. Certificates & Credits for 2 people are included with the purchase of the program, additional Certificates and Credits are available for $150 each to registered users and their employees. (*Note - a new test was posted August 28, 2006 to accompany the new version of the manual. If you have an older version please contact us for the correct test).

  • Laser & Light Based Hair Removal - 16.1 Contact Hours and Certificate of Laser Hair Removal Training (didactic only). Includes Registration sheet. Certificates and Credits for 2 people are included with the purchase of the manual on CD-ROM.
